02About Bloom & our services

Bloom Home Care Services provides non-medical, in-home care services to individuals across Arizona. The information on this Site is provided for general informational purposes only and does not create a care relationship or any contract for services.

Any home care services we provide are subject to a separate written service agreement between you (or your authorized representative) and Bloom, which sets out the specific services, schedule, rates, and terms. If there is any conflict between that service agreement and these Terms, the service agreement controls for matters relating to your care.

03Not medical advice

Bloom provides non-medical home care and does not provide medical, nursing, or clinical diagnosis or treatment. Content on this Site is not medical advice and is not a substitute for care from a qualified health professional. Always seek the advice of your physician or another qualified provider with any questions about a medical condition. In an emergency, call 911.

04Eligibility, ALTCS & AHCCCS

Bloom is a contracted provider within the Arizona Long Term Care System (ALTCS), administered by AHCCCS. Coverage of services through ALTCS/AHCCCS or a health plan depends on the member’s eligibility, authorization, and plan rules, which are determined by AHCCCS and the applicable plan — not by Bloom. We can help you understand and navigate the process, but we cannot guarantee eligibility or coverage. Services and availability may vary by location.

05Acceptable use of this website

When using the Site, you agree not to:

• Use the Site for any unlawful purpose or in violation of these Terms
• Attempt to gain unauthorized access to the Site or its systems
• Interfere with or disrupt the Site’s operation or security
• Submit false information or impersonate another person
